Dr. Prazak began her long and impactful
career with Outer Cape Health Services
in 1989 as a staff physician in Wellfleet
and Provincetown.
Previously she served as interim medical
director of Health Associates of Provincetown
amongst other professional positions on
Cape Cod and Cambridge.
During her tenure as OCHS medical director,
Dr. Prazak was named Physician of the Year in 2015 by the Massachusetts League of Community Health Centers.
A professional mentor to many over the years,
Dr. Prazak is also a beloved and respected
member of Wellfleet and OCHS communities,
and a trusted physician for grateful patients
and generations of families on the
Outer Cape.
She officially retires from OCHS as a
Primary Care Physician on October 14, 2021.
